The Respiratory Disease Education Program is for adults with Asthma, Chronic Obstructive Pulmonary Disease (COPD), and other respiratory diseases.

How to access

  • Check eligibility

    ​We serve adults with COPD, asthma, or other respiratory diseases. 

  • Get a referral

    We accept referrals from any health care provider. Have your health care provider complete the North Shore Chronic Disease Service referral form and fax the completed form to (604) 297-9681.
