The referring clinician assumes the role of Most Responsible Provider (MRP) and is accountable for the patient’s care throughout the treatment period. The MRP must:
-
Be available during clinic hours to address questions, concerns, or urgent issues, including the need for escalation to a higher level of care.
-
Provide direct contact information for themselves or a designated covering provider if they are unavailable.
-
Submit new ambulatory orders in Cerner and renew consent annually, as referrals are valid for up to one year.
NOTE: If the ordering MRP changes, a new consent form must be completed and signed by both the patient and the new MRP.
